• Save Our Seniors (SOS): The SOS program seeks to meet the growing needs of low-income seniors by providing case management and delivering healthy food, toiletries/personal care items and cleaning supplies each month to the seniors' homes.

• Case Management: When our clients receive services from Circle of Care, St. Louis, they will be in the good hands of someone from our Case Management team. Our case managers work to assess, advocate for, and help plan services around our clients' medical and non-medical needs. As their needs change, the case manager will continue to work with them to adjust their services.

• Medicare Counseling: Trained counselors provide individuals with no-cost, confidential counseling and unbiased information regarding Medicare, Medicaid, health insurance, and prescription drug options.

​

• Soulful Wellness: Incorporates evidence-based disease and disability prevention programs to empower adults to self-manage for quality of life. Soulful Wellness staff and volunteers are committed to building a culture of wellness, where individuals are engaged, educated, and empowered to achieve total health.

• My Ride Connection:  Enhances the health and quality of life for seniors and adults with disabilities by providing the comfort and peace of mind of affordable, passenger-focused transportation services through reliable, volunteer drivers.

• In-Home Services: The in-home care services program encompasses advanced personal care, non-medical personal care, and household care provided by experienced, licensed staff. Circle of Care, St. Louis also provides Consumer Directed Care Services for disabled adults who want to direct their own care and hire their own care workers.

• PEARLS: The Program to Encourage Active, Rewarding Lives for Seniors (PEARLS) is a counseling program for seniors that teaches participants how to manage depression. PEARLS uses an in-home counseling model, which consists of counseling sessions followed by a series of follow-up phone calls. The counseling covers three behavioral approaches to depression management: (1) teaches participants to recognize symptoms of depression and understand the link between unsolved problems and depression; (2) helps participants meet recommended levels of social and physical activity; and (3) helps participants identify and participate in personally pleasurable activities. In addition to the in-home counseling and follow-up phone calls, the PEARLS program assesses other factors in the participants’ lives to ensure that other potential factors contributing to depression, such as chronic medical conditions, are adequately treated.​​​
