Community Wellness Program

The Community Wellness Program at Circle of Care St. Louis is a comprehensive initiative aimed at enhancing the well-being and independence of seniors and adults with disabilities. Managed by our skilled staff and supported by a passionate team of volunteers, we offer a range of services that help participants stay in their homes, lead fulfilling lives, and actively engage with their community.

Program Services:

  • Health-Wellness Education: Evidence-based health education programs that empower participants to make informed lifestyle choices for better health.

  • Medicare & Medicaid Counseling: Personalized guidance to help participants optimize their benefits and navigate healthcare options.

  • Volunteer Driver Transportation (My Ride Connection): A safe, reliable transportation service that assists participants with essential travel needs, ensuring their mobility and access to community activities.

  • Problem-Solving Therapy (PEARLS Counseling): Mental health support through practical coping strategies, aimed at improving emotional well-being.

  • Telephone Reassurance: Regular check-in calls to monitor well-being, provide companionship, and reduce social isolation.

  • Supplemental Need Boxes: Distribution of essential health and wellness items directly to participants to support their daily needs.

  • VITA Tax Services: Free tax preparation assistance provided by IRS-certified volunteers to help eligible individuals maximize financial benefits.

  • Heart Health Empowerment Series: A culturally tailored 12-session course aimed at reducing heart disease, particularly within the African American community.

  • Horticultural Therapy: Therapeutic gardening activities designed to improve motor skills, sensory stimulation, and mental health.

  • Cognitive Stimulation Group: Activities and exercises facilitated by volunteers to enhance cognitive function and memory.

  • Caregiver Education: Resources, training, and support to empower caregivers in providing effective care.

  • Support Groups: Safe and welcoming spaces for participants to connect, share experiences, and receive emotional support, fostering a sense of community and resilience.

  • Digital Empowerment Lab: Training and access to digital tools for seniors and adults with disabilities, improving their technology skills and fostering independence.​
